Job summary

Signify Health, part of CVS Health®, is seeking a Per Diem Physician to perform in-home health evaluations, engaging with patients in their own homes and bridging gaps in care. You will work within a national clinician network to provide diagnostic and preventive services, including visits at patients' homes and occasional virtual assessments.

The role offers part-time hours, travel within licensed states, and comprehensive benefits for per diem clinicians.

Qualifications

• Active, unrestricted medical license in the coverage area.
• Multi-state licenses preferred.
• Able to cover Williamsport region.

Responsibilities

• Provide in-home health evaluations and build trusted patient relationships.
• Engage with members to connect them with the right care and resources.
• Conduct virtual visits when needed and document findings.
• Fulfill role requirements across Signify Health services including diagnostic and preventive care.
