Job summary

Signify Health, part of CVS Health, seeks a Per Diem Physician to provide in-home health evaluations, visiting patients in their homes and bridging gaps in care with a clear, patient-centered approach.

You’ll be part of a national clinician network, travel locally, and conduct virtual visits as needed. The role emphasizes compassionate care, safety, and quality while delivering diagnostic and preventive services.

Qualifications

• Active, unrestricted MD or DO license in coverage area(s).
• Multi-state licenses are preferred.

Responsibilities

• Provide in-home health evaluations with patient-specific assessments.
• Perform medical history review and physical evaluation during visits.
• Utilize iPad and clinical workflow; coordinate with health plans as needed.
• Travel locally and within licensed states; conduct virtual visits as needed.
• Provide diagnostic and preventive services per patient plan.
• Support patients and families, bridging gaps to appropriate care.
