Job summary

Atara Home Care is seeking a Per Diem Physical Therapist to provide in-home services across Manhattan, Bronx, Queens, and Westchester. You will select your service borough and work with patients recovering from surgery or managing chronic conditions, delivering personalized therapy in the patient’s home.

Responsibilities include assessments, plan development, and education, with documentation and collaboration with the care team.

Qualifications

• Valid license to practice as a Physical Therapist in NY.
• Preferred experience in home care; not required.
• Excellent communication and interpersonal skills.
• Ability to work independently with a variable schedule.
• Strong time management and organizational skills.

Responsibilities

• Conduct thorough in-home assessments of PT needs.
• Develop and implement individualized treatment plans.
• Provide hands-on therapy and patient education at home.
• Monitor progress and adjust plans as needed.
• Collaborate with other healthcare professionals for comprehensive care.
• Comply with regulatory and company policies.
