Job summary

Personal Touch Home Care in Queens, NY seeks a compassionate Physical Therapist to deliver direct therapy in patients' homes, based on physician plans of care, with focus on improving mobility, function and independence in daily life.

You will assess patients, develop and adjust care plans, evaluate home environments, instruct patients and caregivers in therapeutic exercises, and maintain complete progress and discharge records.

Qualifications

• Must hold a valid NY physical therapy license.
• Excellent communication and organizational skills.

Responsibilities

• Provide direct physical therapy services in the home based on physician plan of care.
• Assess, develop and update care plans to improve mobility and independence.
• Evaluate home environments and educate patients/caregivers on exercises.
• Maintain detailed treatment notes, progress and discharge planning.
