Job summary

Metro Physical & Aquatic Therapy in Eastern Suffolk County seeks a motivated per diem Home Care Physical Therapist to provide in-home services. Flexible schedule, with 1099 and W-2 options to fit your preferred work style, and a patient-centered care ethos.

You will evaluate patients, develop plans of care with physicians and families, and deliver therapy to restore function. Training and mentorship are available; travel to patients’ homes within the territory is required; prior home health

Qualifications

• Licensed Physical Therapist in the state of New York or eligible.
• Home health experience is preferred but not required; mentor provided.
• Ability to travel to patients’ homes within the assigned geographic territory.

Responsibilities

• Accept and treat patients within your designated geographic area.
• Evaluate patients’ needs for physical therapy in the home setting and develop individualized treatment plans.
• Develop a plan of care and treatment goals in consultation with the physician, patient, and family.
• Plan, prepare and implement customized therapy programs to restore function and promote independence.
• Instruct family members, PTAs, Home Health Aides, and caregivers on appropriate therapy techniques.
• Provide timely and accurate documentation for each visit.
• Communicate effectively with patients, families, and interdisciplinary team members, especially around discharge planning.
• Deliver care in compliance with agency policies and professional practice standards.
