Job summary

Mass General Brigham’s Hospital Medicine team in Northampton, MA seeks a Per Diem Nurse Practitioner or Physician Assistant to join as a swing shift admitting provider from 2:00pm to 12:00am. The role includes cross-coverage, coordination with ED and inpatient teams, and involvement in discharge planning on the CDH Main Campus in Northampton.

Qualifications

• Master’s Degree Physician Assistant required.
• Experience as a licensed Physician Assistant preferred.
• Prior APP experience in inpatient medicine required.
• Prior experience with EPIC EMR preferred.

Responsibilities

• Perform initial patient evaluations, including history, medication reconciliation, physical exam, diagnostic review, and documentation of H&P with admission orders.
• Monitor patient progress and coordinate care plans with attending physicians and interdisciplinary teams.
• Document ongoing care through progress notes.
• Perform procedures in accordance with clinical privileges.
• Manage discharges, including medication reconciliation and coordination of follow-up care to support safe transitions.
• Educate patients and families on diagnoses, medications, procedures, and post-discharge care.
• Triage and facilitate patient transfers from the CDH Emergency Department, outside hospitals, and outpatient settings.
• Communicate with accepting providers to ensure seamless transitions of care.
• Participate in Goals of Care / Hospice care discussions with patients and families.
