Mass General Brigham Medical Group Western Massachusetts, Inc. is seeking a Full Time NP or PA to join our Hospitalist team at Cooley Dickinson Hospital in Northampton, MA. The swing shift runs 2:00pm to 12:00am, with inpatient admissions from the Emergency Department.

The NP/PA will work with physicians and care teams to admit patients, perform initial evaluations, coordinate plans, and educate families. Prior inpatient experience is preferred, and EPIC is a commonly used EMR.

Qualifications

• Master’s degree in nursing required.
• APRN-NP state license required.
• 0–1 year NP experience preferred.
• Inpatient medicine experience preferred.

Responsabilités

• Performs initial patient evaluations, including history, medication reconciliation, physical exam, diagnostic review, and documentation of H&P with admission orders.
• Monitors patient progress and coordinates care plans with attending physicians and interdisciplinary teams.
• Documents ongoing care through progress notes.
• Performs procedures in accordance with clinical privileges.
• Manages discharges, including medication reconciliation and coordination of follow-up care.
• Educates patients and families on diagnoses, medications, procedures, and post-discharge care.
• Triages and facilitates patient transfers from the Emergency Department and other settings.
• Communicates with accepting providers to ensure seamless transitions of care.
• Participates in Goals of Care / Hospice care discussions with patients and families.
