Cooley Dickinson Hospital in Northampton, MA seeks a full-time Nurse Practitioner to join its Hospitalist team for a swing shift (2:00pm–12:00am). The NP will admit patients from the Emergency Department to inpatient medical floors and collaborate with physicians, nurses, and case managers to provide high-quality care.

The role emphasizes autonomy in a collegial setting with cross-coverage for hospitalized patients and opportunities to serve as a first responder for rapid assessments.

Qualifications

• Experience as a Nurse Practitioner 0-1 year preferred

Responsabilités

• Perform initial patient evaluations, including history, medication reconciliation, physical exam, diagnostic review, and documentation of admission notes.
• Monitor patient progress and coordinate care plans with attending physicians and interdisciplinary teams.
• Document ongoing care through progress notes.
• Perform procedures in accordance with clinical privileges.
• Manage discharges, including medication reconciliation and follow-up care for safe transitions.
• Educate patients and families on diagnoses, medications, procedures, and post-discharge care.
• Triage and facilitate patient transfers from the ED and other settings.
• Communicate with accepting providers to ensure seamless transitions of care.
• Participate in Goals of Care / Hospice discussions with patient and families.
