Hospitalist NP: Admitting & Rounding (Swing Shift)

Job summary

Cooley Dickinson Hospital in Northampton, MA seeks a Full Time Nurse Practitioner to join the Hospitalist team. This 40-hour position is swing shift, roughly from 2:00 PM to 12:00 AM, admitting patients from the Emergency Department to the Inpatient Medical Floors.

The NP will collaborate with Swing and Nocturnist physicians, provide cross-coverage, perform initial evaluations, document care, coordinate discharge planning, and educate patients and families. Experience 0-1 year preferred.

Qualifications

• Experience as a Nurse Practitioner 0-1 year preferred.

Responsibilities

• Perform initial patient evaluations, including history, medication reconciliation, physical exam, diagnostic review, and documentation of H&P with admission orders
• Monitor patient progress and coordinate care plans with attending physicians and interdisciplinary teams
• Document ongoing care through progress notes
• Perform procedures in accordance with clinical privileges
• Manage discharges, including medication reconciliation and coordination of follow-up care to support safe transitions
• Educate patients and families on diagnoses, medications, procedures, and post-discharge care
• Triage and facilitate patient transfers from the CDH Emergency Department, outside hospitals, and outpatient settings
• Communicate with accepting providers to ensure seamless transitions of care
• Participate in Goals of Care / Hospice care discussions with patient and families.
