Athena Hospice of Rhode Island is seeking a per diem Hospice Social Worker to support Rhode Island residents. You will assess psychosocial needs, counsel patients and families, and coordinate care with an interdisciplinary team.

The role requires a Bachelor's in Social Work and a state license, plus driving. Up to $45 per hour with weekly pay and benefits including life insurance and a 401(k) match.

Qualifications

• Bachelor's in social work or related field.
• State social work license required.
• One year of clinical healthcare and/or hospice experience preferred.
• Understanding of hospice philosophy and issues of death/dying preferred.
• Must be a licensed driver with an insured automobile.
• English language reading, writing, speaking and understanding required.
• Ability to make independent decisions and interact tactfully with others.

Responsabilités

• Assess the psychosocial status of patients and families/caregivers.
• Provide an assessment of the patient's identified residence.
• Carry out social evaluations and plans based on evaluation findings.
• Counsel patients and family/caregivers as needed in relation to stress and coping.
• Educate the team on any special needs related to the culture of the patient and family.
• Participate in the development of the individualized plan of care and attend interdisciplinary meetings.
• Assist the physician and other team members in understanding social and emotional factors related to health problems and death/dying.
