Job summary

Athena Hospice of Rhode Island is seeking a per diem Hospice Social Worker to join our Rhode Island team. You will plan, organize, and direct social services to meet residents' needs, focusing on emotional and social well-being and coordinating with clinical staff.

Responsibilities include psychosocial assessments, family/caregiver support, care plan development with patients and families, and education of the care team on culture and coping.

Qualifications

• Bachelor's in social work or related field required.
• State social work license required.
• One year healthcare/hospice experience preferred.
• Understanding of hospice philosophy and death/dying issues preferred.
• Must be a licensed driver with an insured automobile.
• Must read/write/speak English fluently.

Responsibilities

• Assess the psychosocial status of patients and families/caregivers.
• Provide an assessment of the patient's identified residence.
• Carry out social evaluations including family dynamics and caregiver abilities; plan intervention.
• Counsel patients and families about stress and coping difficulties.
• Educate the team on cultural needs related to patient/family.
