Job summary

Athena Hospice of Rhode Island is seeking a per diem Hospice Social Worker to provide psychosocial support for residents across Rhode Island. This role involves planning, organizing, and directing social services to meet individual patient needs, with a focus on emotional well-being and end-of-life care.

Responsibilities include assessing psychosocial status, conducting family-oriented evaluations, counseling patients and families, and collaborating with the interdisciplinary team to implement

Qualifications

• Bachelor's in social work or related field required.
• State social work license required.
• One year of clinical healthcare and/or hospice experience preferred.
• Understanding of hospice philosophy and issues of death/dying preferred.
• Must be a licensed driver with insured automobile.
• Must be able to read, write, speak, and understand English.
• Ability to make independent decisions when warranted.
• Ability to deal tactfully with residents, families, staff, and public.

Responsibilities

• Assess the psychosocial status of patients and families/caregivers.
• Provide an assessment of the patient's identified residence.
• Carry out social evaluations, including family dynamics and caregiver abilities.
• Counsel patients and family/caregivers as needed regarding stress and coping.
• Educate the team on hospice-related cultural needs.
• Participate in developing the individualized plan of care with patient and family.
• Assist the physician and team in understanding social and emotional factors related to health problems and death/dying.
