Athena Hospice of Rhode Island is seeking a per diem Hospice Social Worker to plan, organize, and direct social services for residents across Rhode Island. Compensation up to $45 per hour with weekly pay, with up to 20 hours per month.

The role requires a license and education in social work, with responsibilities spanning psychosocial assessments, family education, and collaboration with the care team to address culture and end-of-life needs.

Competenze

• Bachelor's in social work or related field required.
• State social work license required.
• 1 year clinical healthcare and/or hospice experience preferred.
• Must be able to read, write, speak English clearly.
• Must possess a driver’s license with insured vehicle.

Mansioni

• Assess the psychosocial status of patients and families/caregivers.
• Provide an assessment of the patient's identified residence.
• Carry out social evaluations, including family dynamics, caregiver abilities, communication patterns, and plans intervention based on evaluation findings.
• Counsel patients and family/caregivers as needed in relation to stress and other identified coping difficulties.
• Educate the team on any special needs related to the culture of the patient and family.
• Participate in development of the individualized plan of care with patient and family; attend interdisciplinary meetings.
• Assist the physician and team in understanding social and emotional factors related to health problems and death/dying.
