Athena Hospice of Rhode Island is seeking a per diem Hospice Social Worker to plan and direct social services for residents across Rhode Island. You will address emotional and social needs, collaborating with the care team to develop care plans and support patients and families.

Qualifications

• Bachelor's in social work or related field required.
• State social work license required.
• One year of clinical healthcare or hospice experience preferred.
• Understanding of hospice philosophy and death/dying issues preferred.
• Must be a licensed driver with insured automobile.
• Must read, write, speak and understand English.
• Ability to make independent decisions when needed.
• Ability to interact tactfully with residents, families, staff and the public.

Responsibilities

• Assess the psychosocial status of patients and families/caregivers.
• Provide assessments of patient residence and social evaluations.
• Carry out social evaluations including family dynamics and caregiver abilities.
• Counsel patients and families regarding stress and coping.
• Educate the team on patient/family cultural needs.
• Participate in development of the individual plan of care with patient and family.
• Assist clinicians in understanding social and emotional factors related to health, death and dying.
