Job summary

Dierksen Hospice is seeking a Licensed Medical Social Worker (LMSW) to deliver compassionate social work services to our hospice patients and families. You will assess psychosocial status, coordinate care with the RN Case Manager and interdisciplinary team, and provide counseling to support emotional well-being during end-of-life care.

The role offers a per diem schedule and coverage across Craighead, Greene, Jackson and Poinsett counties in Arkansas, with a mission-driven, growth-oriented

Qualifications

• Education requirements for LMSW/MSW positions in hospice social work.
• Supervision by MSW for bachelor-level applicants as required.

Responsibilities

• Be a supportive guide by assessing psychosocial status and communicating with care teams.
• Evaluate medical needs, home situations, and available community resources.
• Develop and implement individualized social work plans with bereavement coordination.
• Participate in interdisciplinary team meetings to shape care plans.
• Provide compassionate counseling to patients and families.
