Job summary

Crescent Hospice is seeking a dedicated Licensed Master Social Worker (LMSW) or MSW to deliver compassionate social work services for hospice patients and families. This per diem role focuses on psychosocial support, counseling, and coordinating with the RN Case Manager and interdisciplinary team.

The position covers Rock Hill, Fort Mill, Lancaster and Chester Areas with a strong emphasis on patient-centered care and a supportive onboarding program.

Qualifications

• Master of Social Work from an accredited school of social work is required.
• Licensed as a Social Worker by the State Board of Social Work is required.
• Current Driver's License, vehicle insurance, and access to a dependable vehicle are required.
• Preferred: Minimum one year hospice social work experience; required: minimum one year in a healthcare setting (LTAC/acute care or similar).

Responsibilities

• Assess and communicate psychosocial status to the RN Case Manager and interdisciplinary team.
• Evaluate medical needs, home situations, financial resources, and community support.
• Implement personalized social work plans and collaborate with the Bereavement Coordinator.
• Shape care plans with the interdisciplinary team and address psychosocial stresses.
• Provide empathetic counseling to patients and families.
