Job summary

Constellation Health Services seeks a compassionate Hospice Registered Nurse to provide per diem/PRN care in Orange, CT and neighboring areas. The role covers on-call shifts, home visits, and coordination with families and interdisciplinary teams.

Flexible scheduling includes weekdays and weekends with mileage documentation included. Requirements include RN licensure in CT and 1–2 years in hospice; comfort with iOS documentation and travel between patient sites.

Qualifications

• Graduate of an accredited RN program.
• Registered Nurse Licensed in the state of employment.
• At least 1-2 years of RN experience in hospice preferred.
• Basic knowledge of iOS operating system.

Responsibilities

• Provides skilled nursing care including administration of medications and treatments in patients' homes or facilities.
• Performs comprehensive assessments and coordinates care with multi-disciplinary team.
• Observes and reports changes in patient status to nursing supervisor and physician.
• Maintains documentation in agency system and ensures timely submissions.
• Participates in weekly interdisciplinary team meetings and family communications.
