Job summary

Waveny Brand in Connecticut is seeking a dedicated Registered Nurse to oversee hospice patient care delivery, coordinate referrals, and ensure compliance with agency documentation standards. You will provide pain and symptom management, participate in IDG team meetings, supervise home visits, and support families while upholding hospice philosophy and professional nursing practice.

Qualifications

• Current license to practice as RN in Connecticut.
• Valid driver's license.
• Minimum three years clinical experience in past five years, at least one in palliative/hospice/long-term care.
• Understanding of hospice principles and philosophy.
• Ability to manage terminal care with pain and symptom management.

Job description

Implements, coordinates and supervises all aspects of delivery of patient care for hospice program:

• Triages incoming calls for referral or care and implements appropriate plan (i.e. home visit, professional consultation, interagency referral, etc.)
• Complies with Agency documentation standards.
• Provides consultation and visits when appropriate for pain management, symptom management, comfort care, HHA orientations/supervisions, Death Pronouncement and bereavement care
• Maintains ongoing communication with IDG staff.
• Calls in report before 8:30 a.m. to all appropriate parties on patient issues/contacts during evening/night.
• Seeks consultation with other agency staff as indicated.
• Participates in Hospice IDG Team and other meetings, as requested.

Educational

• Certified in CPR
• Attends yearly OSHA in-service and completes annual skills validation
• Identifies knowledge gaps and remedies independently
• Attends minimum 12 hours of in-services per year, including 6 hours of hospice in-service.
• Current license to practice as RN in Connecticut.
• Valid driver's license
• Minimum three years clinical experience in the past five years, at least one of which must be palliative care, end of life care, long-term care, or hospice care.
• An understanding of and commitment to hospice principles and philosophy.
• Demonstrated ability to manage all aspects of terminal care with expertise in pain and symptom management and hospice nursing.
