Qualifications

• Graduate of an accredited Nurse Practitioner program.
• Active Connecticut APRN license with prescriptive authority.
• 2–3 years of clinical experience preferred.
• Experience in hospice, palliative care, geriatrics, or home-based care preferred.

Responsabilités

• Conduct face-to-face visits for hospice eligibility and recertification.
• Provide on-call coverage and triage for hospice patients, minimum 4 weeknights per month.
• Participate in weekly Interdisciplinary Team (IDT) meetings with clinicians and support staff.
• Offer clinical guidance and consultation to field staff.
• Communicate prognosis and plan of care to patients, families, and caregivers.
