Job summary

Care New England’s VNA in Warwick, RI is hiring a Registered Nurse - Hospice for per diem, on-call home-based care. You’ll join an interdisciplinary team delivering compassionate, patient-centered hospice services in patients’ homes.

Qualifications

• Active RI Registered Nurse license required.
• Bachelor’s degree in Nursing strongly preferred.
• At least 1 year of clinical nursing experience; hospice preferred.
• Current CPR certification and valid driver’s license.

Responsibilities

• Manage a caseload of hospice patients within home settings.
• Assess, plan, and coordinate patient care with an interdisciplinary team.
• Monitor and adjust treatment to promote optimal outcomes.
• Educate patients and caregivers on managing conditions at home.
• Document clinical notes with accuracy and clarity.
• Coordinate referrals across disciplines and specialties.
