Job summary

Waveny Care Network is seeking a Registered Nurse with Connecticut licensure to lead and coordinate hospice care delivery. Responsibilities include triaging referrals, planning home visits, and supporting pain and symptom management for patients at end of life.

The ideal candidate has at least three years of recent clinical experience in hospice, palliative, or long-term care, and a deep commitment to hospice principles and compassionate care.

Qualifications

• Active CT RN license.
• Minimum three years clinical experience in the past five years, with hospice/palliative care focus.

Responsibilities

• Triage incoming referrals and coordinate care plans (home visits, consultations, referrals).
• Ensure adherence to agency documentation standards.
• Provide pain management and symptom management visits; support bereavement care and death pronouncement as needed.
• Maintain ongoing communication with IDG staff; call in report before 8:30 a.m. to relevant parties.

Job description

Implements, coordinates and supervises all aspects of delivery of patient care for hospice program:

• Triages incoming calls for referral or care and implements appropriate plan (i.e. home visit, professional consultation, interagency referral, etc.)
• Complies with Agency documentation standards.
• Provides consultation and visits when appropriate for pain management, symptom management, comfort care, HHA orientations/supervisions, Death Pronouncement and bereavement care
• Maintains ongoing communication with IDG staff.
• Calls in report before 8:30 a.m. to all appropriate parties on patient issues/contacts during evening/night.
• Seeks consultation with other agency staff as indicated.
• Participates in Hospice IDG Team and other meetings, as requested.

Educational

• Certified in CPR
• Attends yearly OSHA in-service and completes annual skills validation
• Identifies knowledge gaps and remedies independently
• Attends minimum 12 hours of in-services per year, including 6 hours of hospice in-service.
• Current license to practice as RN in Connecticut.
• Valid driver’s license
• Minimum three years clinical experience in the past five years, at least one of which must be palliative care, end of life care, long-term care, or hospice care.
• An understanding of and commitment to hospice principles and philosophy.
• Demonstrated ability to manage all aspects of terminal care with expertise in pain and symptom management and hospice nursing.
