Job summary

Amedisys, part of the Optum family, is seeking a Per diem Hospice Registered Nurse in Massachusetts. You will assess patients, coordinate care plans, manage symptoms, and educate families while collaborating with the clinical team.

Coverage includes Athol and surrounding areas with orientation at the Leominster care center. Per diem schedule with on-call responsibilities and a comprehensive benefits outlook are provided where eligible.

Qualifications

• Active RN license in assigned state(s).
• Current CPR certification.
• Valid driver's license with reliable transportation and auto liability insurance.
• Preferred: 1+ year nursing experience.

Responsibilities

• Performs initial nursing assessment on new admissions.
• Coordinates and revises the plan of care with the care team.
• Communicates patient progress to the clinical manager, physician, and care team.
• Prepares and maintains individualized patient care progress records.
• Records pain/symptom management changes/outcomes.
• Educates families on patient condition, needs and care.
• Makes referrals to other hospice disciplines as needed.
• Performs on-call responsibilities for patients/families.
• Performs other duties as assigned.
