Amedisys is seeking a compassionate Per diem Hospice Registered Nurse to join our patient-centered team in Leominster, MA. You will assess patients, coordinate care plans, manage symptoms, and educate families in home hospice settings.

Responsibilities include initial nursing assessments, updating care plans, communicating progress with the clinical team, providing on-call coverage, and ensuring high-quality symptom control in alignment with policy and patient goals.

Qualifications

• Current RN license valid for the state you work in (MA preferred).
• CPR certification current and maintained.
• Valid driver's license with reliable transportation.

Responsabilités

• Performs initial nursing assessment on new admissions.
• Coordinates and revises the plan of care with the clinical team.
• Regularly communicates patient progress to the care team.
• Develops and maintains individualized care progress records.
• Records pain/symptom changes and educates families.
• Provides on-call coverage as assigned.
