Job summary

UnitedHealth Group, through its hospice operations, is seeking a Per Diem RN Case Manager to coordinate compassionate hospice care by assessing patients and partnering with the interdisciplinary care team.

You will develop individualized care plans, document patient progress, educate families, and participate in on-call duties. Requires active RN license, CPR, valid driver’s license, and 1+ year of nursing experience. Salary reflects local market and full-time equivalence.

Qualifications

• Current RN license, state-specific as required.
• CPR certification is required.
• Valid driver's license and reliable transportation.
• At least 1 year of nursing experience.

Responsibilities

• Performs patient assessments and collaborates with the care team to develop and implement a plan of care.
• Makes referrals to other hospice care disciplines as needed or documents rationale for not doing so.
• Develops and maintains individualized patient care progress records.
• Records pain and symptom management changes and outcomes.
• Teaches patients, families, and caregivers appropriate patient care techniques.
• Communicates with clinical manager and care team.
• Performs on-call responsibilities and on-call services to patients/families.
