Job summary

Journey Hospice is seeking a per diem RN Case Manager to join a compassionate team dedicated to home-based hospice care in New Jersey. This position is per diem, as needed, offering scheduling flexibility and pay rate up to $47/hour.

The RN will oversee a primary hospice caseload, perform initial and comprehensive nursing assessments, and identify problems. They will implement and revise the plan of care with the Interdisciplinary Team, educate families, and supervise Hospice Aides/Homemakers.

Qualifications

• Graduate of an accredited RN program.
• Current NJ RN licensure.
• CHPN certification preferred.

Responsibilities

• Provide skilled nursing care to hospice patients and families.
• Assess and develop the Hospice Interdisciplinary Team plan of care.
• Educate, supervise, and counsel patient, family, and staff.
• Collaborate with IDT to meet patient and family needs.
