Job summary

Journey Hospice in New Jersey is seeking a per diem RN Case Manager to provide skilled nursing care to Hospice patients with flexible scheduling. The RN will perform assessments, develop and implement the care plan with the interdisciplinary team, and supervise Hospice Aides/Homemakers.

The role requires NJ RN licensure, with CHPN certification preferred. This is an as-needed position offering professional growth and compassionate patient care.

Qualifications

• Strong communication skills and ability to relate courteously and cooperatively.
• Experience working in a hospice or home health setting preferred.

Responsibilities

• Oversee primary caseload of Hospice patients and provide skilled nursing care.
• Perform initial and comprehensive nursing assessments and develop IDT care plans.
• Implement and reevaluate the care plan to meet nursing needs and goals.
• Educate, supervise, and counsel patients, families, and staff on nursing care.
• Collaborate with IDT members to meet patient and family care needs.
