AdvisaCare Hospice in Gilbert, AZ is seeking a compassionate Hospice RN to support patients and families, delivering dignity and comfort at the end of life in the West Valley and surrounding areas. You will perform initial and ongoing assessments, implement and revise plans of care, ensure Medicare compliance, educate families, supervise paraprofessionals, and participate in IDG conferences.

Qualifications

• Current Arizona RN license in good standing.
• Empathetic approach to patients and caregivers.
• Willingness to conduct field visits daily and attend weekly office meetings.
• Ability to adapt to patient status and needs on a full-time on-call basis.

Responsabilités

• Initial and ongoing assessment of patient’s physical, functional, psychosocial and environmental needs.
• Implement the individualized plan of care and revisions as needed.
• Compliance with AdvisaCare Hospice Policies and Procedures, Medicare compliance.
• Consulting with and educating the patient/family on disease process, self-care techniques, end-of-life care.
• Supervision of paraprofessionals providing services to patient.
• Preparing clinical and progress notes, participating in IDG conferences.
• Participate in Hospice performance improvement program.
• Maintain high level of customer satisfaction with our clients, families and caregivers.
• Communication of information using current process and technology within organization.

Are you looking for a challenging and rewarding position as a Hospice RN? We are searching for compassionate, dynamic, energetic, organized and efficient professionals with Hospice experience to become part of our Hospice Family for our Gilbert AZ location servicing the West Valley and surrounding areas!

For this role, you need to be a registered nurse who sees the value of Hospice care. You will be providing emotional support, with the goal to help patients live their final days with dignity and comfort. Our Hospice RN's will conduct appropriate supervisory visits which allow for clinical observation of performance of assigned team members. Participate and provide feedback to the team for regular interdisciplinary group meetings; on the development, review and revision of the patient/family plan of care.
