Job summary

AdvisaCare Hospice is seeking a compassionate Hospice RN for our Gilbert AZ location serving the West Valley and surrounding areas. You will provide emotional support, conduct supervisory visits, and participate in interdisciplinary group meetings to develop and revise patient/family care plans.

The role requires an active Arizona RN license in good standing and a patient-centered approach to end-of-life care. We offer a rewarding work environment with strong support and benefits.

Qualifications

• Current Arizona License in good standing.

• Ability to participate in field visits daily and attend weekly office meetings.

Responsibilities

• Initial and ongoing assessment of patient’s needs across physical, functional, psychosocial, and environmental domains.
• Implement and revise the plan of care; ensure compliance with policies and Medicare guidelines.
• Educate patients and families on disease process, end-of-life care, and self-care techniques.
• Supervise paraprofessionals delivering services and participate in IDG conferences.
• Document progress notes and participate in performance improvement activities.
• Maintain high customer satisfaction with clients, families, and caregivers.

We are searching for compassionate, dynamic, energetic, organized and efficient professionals with Hospice experience to become part of our Hospice Family for our Gilbert AZ location servicing the West Valley and surrounding areas!

For this role, you need to be a registered nurse who sees the value of Hospice care. You will be providing emotional support, with the goal to help patients live their final days with dignity and comfort. Our Hospice RN's will conduct appropriate supervisory visits which allow for clinical observation of performance of assigned team members. Participate and provide feedback to the team for regular interdisciplinary group meetings; on the development, review and revision of the patient/family plan of care.

Job Responsibilities:

• Consulting with and educating the patient/family on disease process, self-care techniques, end-of-life care, dealing with issues of ethical concern
• Supervision of paraprofessionals providing services to patient
• Preparing clinical and progress notes, participating in IDG conferences
• Participate in Hospice performance improvement program
• Maintain high level of customer satisfaction with our clients, families and caregivers
• Communication of information using current process and technology within organization
