Job summary

Bristol Hospice - Hawaii LLC in Honolulu, HI is seeking a compassionate Per Diem Nurse Practitioner to provide patient-centered hospice and palliative care. You will work with an interdisciplinary team to deliver quality end-of-life care with dignity, respect and clinical excellence.

This per diem role offers flexible scheduling, autonomy in patient care, and opportunities for advanced training within a supportive hospice culture.

Qualifications

Must be a graduate of an accredited School of Nursing and accredited Nurse Practitioner Program.
Must have current Hawaii nursing licensure.
Must have a master's degree with at least 1 year NP experience and 1 year hospice/palliative care experience.
Must be accredited by a certifying body for advanced practice nursing as required by Hawaii.
Certification in a specialist area (CHPN, Pediatrics or Geriatrics) preferred.
Must have a licensed driver with insured vehicle.

Responsibilities

Complete hospice face-to-face visit requirements before third and subsequent benefit periods.
Provide medical care and palliate/manage terminal illness under physician supervision.
Perform history and exams, order/interpret tests, document findings.
Develop a plan of care, prescribe medications, obtain consultations, coordinate hospice services.
Assist hospice director with eligibility and recertification review.
Coordinate care with the interdisciplinary team and other practitioners.
Ensure symptom control and review treatment protocols with the team.
Communicate plan of care changes with the team and family.
