Altus Hospice is seeking Per Diem dedicated Hospice Nurse Practitioners to deliver compassionate end-of-life care in the Beaumont area. You will evaluate health histories, diagnose medical conditions, and develop personalized care plans in collaboration with patients and families.

Responsibilities include providing palliation, guiding care with the hospice team, and ensuring compliance with nursing practice acts while maintaining a patient-centered approach.

Qualifications

• Current unencumbered RN/NP license in the State of Practice.

Responsibilities

• Deliver comprehensive hospice medical care under physician supervision.
• Collaborate with hospice team and physicians to assess care needs.
• Coordinate and oversee patient care within interdisciplinary team.
