Job summary

Altus Hospice is currently hiring Per Diem dedicated Hospice Nurse Practitioners to deliver exceptional patient care in and around the Beaumont Area. As a Hospice NP, you will evaluate health histories, diagnose conditions, and develop personalized care plans in collaboration with patients and families.

Qualifications

• Master's degree in nursing with hospice/palliative care experience preferred.
• Current RN/NP license in practice state.
• Excellent verbal and written communication abilities.

Responsibilities

• Deliver comprehensive medical care and palliate terminal illnesses under physician supervision.
• Collaborate with hospice medical director and other practitioners on care needs.
• Coordinate and oversee patient care with interdisciplinary team.
• Educate patients and families on care goals, prevention, and spiritual concerns.
