Job summary

Altus Hospice Care is seeking a Per Diem Hospice Aide (CNA) to join our compassionate team, delivering hands-on, dignified care for patients at the end of life. You will work with our nursing team to address physical, emotional, and personal needs with respect and professionalism.

Responsibilities include following the plan of care, assisting with personal care, delivering outstanding patient service, supporting families, and maintaining precise documentation.

Qualifications

• High School Diploma or GED required.
• Current unencumbered CNA certificate in the state.
• Minimum one year CNA experience in acute or long-term care; hospice a plus.
• CPR certification if state mandates.
• Valid driver’s license with reliable transportation.

Responsibilities

• Provide personalized care following the care plan to ensure patient comfort and dignity.
• Assist with personal care including bathing, hygiene, bedding, clothing, and grooming.
• Maintain exceptional customer service and empathy in all patient interactions.
• Support families by sharing knowledge and guiding them through the care process.
• Maintain accurate documentation in patient records and regulatory compliance.

Responsibilities

• Provide Personalized Care: Follow the carefully crafted plan of care to ensure the comfort and well-being of our patients. Attend promptly to the requests of patients and their family/caregiver, offering compassionate and unwavering support.
• Assist with Personal Care: Support patients with various personal care needs, including bathing, oral hygiene, changing bedding, clothing, and shampooing. Your presence during these intimate moments contributes significantly to their overall comfort and dignity.
• Be Dedicated to Customer Service: Emphasize exceptional customer service, going above and beyond to ensure optimal patient satisfaction. Your dedication to providing outstanding care will be valued and celebrated as part of our team.
• Provide Support for Families: Impact patients' families by sharing your knowledge and expertise, guiding and supporting them in caring for their terminally ill loved ones. Your presence offers confidence and reassurance during this challenging time.

Qualifications

• High School Diploma or G.E.D. required.
• Holds current unencumbered certificate in the state working as a C.N.A.
• Minimum one (1) year experience as a C.N.A. in an acute care or long term care setting. Hospice exp a plus!
• Possess and maintains current CPR certification if state- mandated
• Current driver's license, vehicle insurance, and access to dependable transportation.

$17.00 - $20.00 per hour (Average Pay Range). The pay range listed represents a general posting guideline for the role and is not a fixed offer. Final compensation will be determined based on the candidate’s relevant experience, qualifications, and the specific responsibilities of the position. The exact compensation rate will be discussed and confirmed at the conclusion of the interview process.
