Job summary

Assisted Home Health & Hospice is seeking a per diem hospice MSW to cover Lompoc and nearby areas. You will assess, plan, and coordinate social work services for patients and families in home hospice settings.

The role emphasizes psycho-social evaluation, collaboration with physicians and the care team, and discharge planning while supporting patient rights and confidentiality. Prior hospice experience is preferred.

Qualifications

• Master of Social Work degree from a school accredited by the CSWE.
• Current CPR, TB and health clearance per skills checklist.
• At least one year of social work experience in a health care setting.

Responsibilities

• Assess and evaluate psycho-social status.
• Develop the plan of care in consultation with the physician and care team.
• Provide interventions to enhance coping for patient and family.
• Assist physician and team in understanding psychosocial factors.
• Document patient progress and report changes to the physician.
• Evaluate home environment and community resource needs.
• Advise and counsel family on psycho-social needs.
• Participate in discharge planning and care coordination.
• Maintain patient confidentiality and professional appearance.
• Abide by agency policy and perform other duties as assigned.
