Job summary

Continuum Hospice & Palliative Care California is seeking a Per Diem Chaplain to provide spiritual and emotional support to patients and families navigating end-of-life care. The role is based in Alameda and Contra Costa Counties, with a schedule of 3 weekdays, 8 am to 4:30 pm, joining an interdisciplinary team dedicated to comfort, dignity, and compassionate presence.

The ideal candidate has interfaith experience, formal theological education (BD/MDiv or equiv.), and is proficient with EMR

Qualifications

• Graduate of an accredited seminary or school of theology (BD, MDiv, or equivalent theological degree) or equivalent pastoral certification.
• Minimum of one unit of Clinical Pastoral Education (CPE).
• Comfortable working in interfaith settings and able to respect diverse beliefs.
• Experience providing spiritual care in hospice, hospital, or community health settings preferred.
• Must be computer proficient with EMR experience.
• Must have a valid driver's license, vehicle insurance, and reliable transportation.
• CPR certification current.

Responsibilities

• Assess and address spiritual and emotional needs of patients and families as part of the IDT care plan.
• Provide spiritual counseling, comfort, and guidance for all faiths.
• Offer grief and bereavement support throughout hospice care and after loss.
• Collaborate with Program Director and care team to coordinate spiritual care.
• Communicate effectively with hospice staff and partners to ensure continuity and quality of care.
• Maintain accurate documentation per regulatory standards.
