Job summary

Continuum Care Hospice is seeking a Per Diem Chaplain to provide spiritual and emotional support to patients and families as they navigate life’s meaningful moments. You will work within an interdisciplinary team to honor all faiths and beliefs, ensuring comfort, dignity, and compassionate presence.

The role involves 3 weekdays, 8 am–4:30 pm, serving Alameda & Contra Costa County, with comprehensive benefits and a supportive onboarding program.

Qualifications

• Graduate of an accredited seminary or school of theology (BD, MDiv, or equivalent) strongly preferred, or equivalent experience.
• Minimum of one unit of Clinical Pastoral Education (CPE) strongly preferred.
• Comfortable working in interfaith settings and able to accept diverse beliefs.
• Experience providing spiritual care in hospice, hospital, or community health setting preferred.
• Must have ability to handle stress when ministering to patients, families, and loved ones.
• Must be computer proficient, including EMR familiarity.
• Current driver's license, vehicle insurance, and dependable transportation.
• Maintain CPR certification at all times.

Responsibilities

• Assess and address spiritual and emotional needs of patients and families as part of the interdisciplinary care plan.
• Provide spiritual counseling, comfort and guidance for individuals and families of all faiths and beliefs.
• Offer grief and bereavement support throughout the hospice journey and after loss.
• Collaborate with Program Director and care team to coordinate spiritual care and holistic support.
• Communicate effectively with hospice staff and community partners to ensure continuity and quality of care.
• Maintain accurate documentation in accordance with regulatory standards and agency policies.

Qualifications

• Minimum of one unit of Clinical Pastoral Education (CPE) strongly preferred.
• Comfortable working in interfaith settings and able to accept diverse beliefs.
• Experience providing spiritual care in a hospice, hospital, or community health setting is strongly preferred.
• Must have the ability to handle stress associated with ministering to patients/residents and their loved ones who experience pain, suffering, death, and grief.
• Must be computer proficient in typing and various programs, including a background in EMR.
• Current driver's license, vehicle insurance, and access to dependable transportation.
• Possess and Maintain current CPR Certification

$41.00 - $44.00 per hour (Average Pay Range). The pay range listed represents a general guideline for the role and is not a guarantee of the final offer. Compensation will be determined based on the selected candidate’s relevant experience and the specific responsibilities of the position. Final compensation rate will be discussed and confirmed at the conclusion of the interview process.
