Job summary

Dartmouth-Hitchcock Health is seeking an APRN to join the Home Health Hospice program, delivering compassionate palliative and end-of-life care to patients in their homes.

The APRN will assess and manage symptoms, educate families, and collaborate with a multidisciplinary team within the Dartmouth Health at Home system.

Current NH/VT licensure and national certification required; fellowship or hospice experience preferred.

Qualifications

• Board-certified APRN with national certification in a relevant NP specialty.
• Eligible for NH and VT nursing licenses and DEA registration.
• Fellowship in palliative care or at least 2 years of hospice/palliative experience expected.

Responsibilities

• Evaluate and manage palliative care needs, including symptom control and goals of care.
• Provide patient and family education aligned with care plans.
• Collaborate with the hospice medical team and other professionals in home-based settings.
• Assist with complex care planning and communication with primary providers.
