Job summary

Endurance Home Care is seeking per diem RNs to perform initial client assessments and 60-day supervisory visits in New Jersey. Responsibilities include writing care plans, meeting with clients and families, and matching clients with suitable CHHAs for in-home care.

Qualifications

• Active New Jersey RN license is required.
• At least 1 year of clinical experience; home care, elder care or community health preferred.
• Reliable transportation and a valid driver’s license.
• Comfortable doing independent in-home visits and documenting electronically.
• Warm, clear communicator with clients and families.

Responsibilities

• Perform in-home nursing assessments for new clients.
• Write individualized care plans based on each client’s needs, goals and home environment.
• Meet with clients and their families to explain the plan of care and answer questions.
• Help match clients with the right caregiver.
• 60-Day Supervisory Visits to review conditions and update care plans.
• Supervise CHHAs and ensure care meets NJ requirements.
• Document visits on time in our digital platform.
