Responsibilities

• Assess patient at start of care to develop a plan of care.
• Initiate Plan-of-Care and adjust as patient needs evolve.
• Evaluate patients in their homes.
• Instruct and advise patients and families as well as home health aides.
• Reassess patients every two months and supervise home health aides to ensure proper care.
