Job summary
Able Home Care LLC is seeking a Per-Diem Home Health Registered Nurse to provide skilled nursing care in patients' homes in the Brockton area. The role emphasizes autonomy, meaningful patient relationships, and flexible per-diem scheduling to fit your life.

Qualifications
- Current RN license in Massachusetts.
- Strong technology skills.
- Reliable transportation with valid driver's license and auto insurance.
- Strong clinical assessment and documentation skills.
Responsibilities
- Delivers skilled nursing care to patients in their home per individualized care plans.
- Initiates preventive and rehabilitative nursing procedures and referrals.
- Coordinates services, informing physicians and staff of changes in patient condition and needs.
- Ensures accuracy of patient records in compliance with Medicare and MassHealth standards.
- Conducts ongoing medical documentation, including OASIS assessments as appropriate.
- Prepares clinical and progress notes and submits required paperwork daily.
