Qualifications

• Current RN license in Massachusetts.
• Strong technology and documentation skills.
• Reliable transportation and valid driver’s license.
• Ability to work autonomously in home health settings.

Responsibilities

• Deliver skilled nursing care in patients' homes per care plans.
• Coordinate care and communicate changes to physicians and team.
• Complete and maintain patient records per Medicare/MassHealth standards.
• Prepare notes and assessments daily; respond to communications promptly.
