Able Home Care LLC is seeking a Per-Diem Home Health Registered Nurse to deliver skilled nursing care in patients’ homes in the Brockton area. The role offers flexible per-diem scheduling, competitive per-visit pay, weekend differential, mileage reimbursement, and autonomy in daily practice.

As an RN, you’ll coordinate care, build trust with patients, and make a measurable impact in a home setting, while maintaining compliance with Medicare/MassHealth standards and thorough documentation.

Qualifikationen

• Current RN license in Massachusetts.
• Strong technology skills.
• Reliable transportation with valid driver’s license and auto insurance.
• Strong clinical assessment and documentation skills.

• Delivers skilled nursing care to patients in their home per individualized care plans.
• Initiates preventative and rehabilitative nursing procedures and makes referrals to community services.
• Coordinates services and informs physicians, management and others of changes in patient condition.
• Ensures quality and accuracy of patient records per Medicare and MassHealth standards.
• Writes OASIS assessments as appropriate.
• Prepares clinical and progress notes and submits required paperwork daily.
• Responds promptly to emails and phone calls.

What You'll do:

As a Home Health RN, you'll deliver skilled, individualized nursing care to patients in the comfort of their own homes- coordinating care, building trust , and making a measurable impact every day.

Key Responsibilities

• Delivers skilled nursing care to patients in their home in accordance with individualized care plans.
• Initiates appropriate preventative and rehabilitative nursing procedures, making necessary referrals to community services.
• Coordinates services, informing the physician, management, and other personnel of changes in the patient's condition and needs.
• Ensures quality and accuracy of patient records in compliance with Medicare and Mass Health standards.
• Conducts ongoing medical documentation, writing OASIS assessments as appropriate.
• Prepares clinical and progress notes, submitting all required paperwork daily. This is a performance requirement.
• Responds promptly to emails and phone calls
