Able Home Care LLC is seeking a Per-Diem Registered Nurse (RN) to provide skilled nursing in patients' homes on the North Shore. You will deliver individualized care, coordinate services, and document care with a focus on autonomy and patient relationships.

Qualifikationen

• Active RN license in Massachusetts
• Strong technology skills
• Reliable transportation with valid driver's license and auto insurance
• Strong clinical assessment and documentation skills

Aufgaben

• Delivers skilled nursing care to patients in their home in accordance with individualized care plans
• Initiates appropriate preventative and rehabilitative nursing procedures and referrals to community services
• Coordinates services and informs physicians, management, and other personnel of changes in patient condition
• Ensures quality and accuracy of patient records in compliance with Medicare and MassHealth standards
• Conducts ongoing medical documentation and writes OASIS assessments as appropriate
• Prepares clinical and progress notes and submits required paperwork daily
• Responds promptly to emails and phone calls
