Job summary

Able Home Care LLC is seeking a Per-Diem Home Health Care Registered Nurse to provide skilled nursing care in patients’ homes on a per-visit basis during the North Shore area. You’ll deliver individualized care, coordinate with physicians, and document care plans while maintaining patient trust and autonomy.

The role offers autonomy, a flexible per-diem schedule, and competitive per-visit compensation with mileage reimbursement.

Qualifications

• Current RN license in the state of Massachusetts
• Ability to work per-diem with flexible scheduling
• Reliable transportation and valid driver’s license

Responsibilities

• Delivers skilled nursing care to patients in their home in accordance with individualized care plans.
• Initiates appropriate preventative and rehabilitative nursing procedures, making necessary referrals to community services.
• Coordinates services, informing the physician, management, and other personnel of changes in the patient’s condition and needs.
• Ensures quality and accuracy of patient records in compliance with Medicare and Mass Health standards.
• Conducts ongoing medical documentation, writing OASIS assessments as appropriate.
• Prepares clinical and progress notes, submitting all required paperwork daily. This is a performance requirement.
• Responds promptly to emails and phone calls
