Qualifications

• Requires graduation from accredited nursing school.
• Current state licensure.
• CPR certification.
• Prefer one year experience in a related field.

Responsibilities

• Provide day-to-day patient care in the home setting.
• Perform post-hospitalization assessments and recertifications.
• Administer IV therapy and wound care as needed.
• Document care using Epic EMR in a timely manner.
• Coordinate with care team to ensure patient safety and appropriate care.
