Mercy is seeking a dedicated Registered Nurse for Home Health per diem visits in Oklahoma. You will assess patients, plan and deliver care across varying acuity levels, and coordinate admissions and discharges from home care services.

Responsibilities include IV therapy, patient teaching, and comprehensive documentation in Epic. A valid RN license, CPR, and driver’s license with reliable transportation are required; home health experience is preferred.

Qualifications

• Current state licensure as a Registered Nurse (RN).
• CPR certification required.
• Valid driver's license and reliable transportation.
• Preferred 1 year of related Home Health experience.
• Graduation from an accredited nursing program.

Responsabilités

• Provide day-to-day patient care in home settings, including assessment and care planning.
• Perform post-hospitalization assessments, recertifications, and discharges.
• Administer IV therapy and manage dressing changes as per policy.
• Educate patients and families, document care in Epic per standards.
