Job summary

TRU Healthcare, Inc is seeking a Home Health Per Diem Physical Therapy Assistant (PTA) to provide skilled therapies in patients’ homes across the Twin Cities Metro area. You will work under the supervision of a Physical Therapist to implement treatment plans and promote mobility, strength, balance and independence.

The role emphasizes travel to client homes with mileage reimbursement. Per diem, per-visit compensation supports flexible scheduling and ongoing patient-centered care.

Qualifications

• Must hold valid MN PTA license.
• Previous home health and/or outpatient experience strongly preferred.
• Able to travel around the Twin Cities Metro area to provide services (mileage paid).

Responsibilities

• Provide skilled physical therapy services in patients' homes by implementing treatment plans established by the supervising PT, helping patients improve mobility, strength, balance, and independence.
• Monitor patient progress and document care accurately and promptly, communicating changes in condition to ensure high-quality outcomes.
• Educate patients on home exercise programs, safe mobility techniques, fall prevention, and proper use of assistive devices.
