Job summary

Beth Israel Lahey Health at Home is seeking an experienced Physical Therapist to coordinate and manage patient care across Burlington, Lexington, Lincoln, and Waltham. The role emphasizes collaboration with PTA staff, supervising care, and developing comprehensive plans of care that involve patients and families.

You will administer prescribed treatments, provide patient education, and ensure timely documentation while supporting discharge planning and interdisciplinary coordination.

Qualifications

• Licensed Physical Therapist in Massachusetts.
• Active American Heart Association BLS or equivalent.
• Graduate of an accredited Physical Therapy program; DPT preferred.
• OASIS experience is preferred.

Responsibilities

• Coordinates patient care for assigned caseload, delegating to PTA staff and supervising care.
• Monitors patient satisfaction scores and quality of care outcomes.
• Completes ante/physical assessment and histories of illnesses.
• Develops patient-specific plans of care with goals and therapeutic actions.
• Involves patient and family in planning and makes revisions as needed.
• Administers prescribed treatments and counsels patient and family.
• Provides care instructions and educates on therapy plans.
• Supervises home health aides and evaluates care every two weeks.
• Coordinates discharge planning and communicates with physician as needed.
• Participates in case conferences and team meetings.
• Communicates with team PTAs and other disciplines to coordinate care.

Massachusetts licensure as Physical Therapist
