Job summary

Beth Israel Lahey Health at Home is seeking a licensed Physical Therapist to coordinate patient care across home health teams in Massachusetts. You will supervise PTA staff, manage OASIS-completed visits, and develop patient-centered plans of care with family involvement.

The role requires MA licensure and a DPT preferred, with 1–2 years in home health or public health settings. Flexibility to work a 24-hour schedule in a community-based program is provided.

Qualifications

• Physical Therapist with MA licensure to practice therapy.
• Active BLS as required by policy.
• Graduate of an accredited PT program; DPT preferred.

Responsibilities

• Coordinate patient care for assigned caseload with PTA supervision.
• Administer medications and treatments as prescribed; counsel patient and family.
• Develop and revise plan of care with patient and family involvement.
• Instruct and supervise home health aides every two weeks.
• Coordinate with physicians and other disciplines; attend case conferences.
• Complete OASIS and clinical visit notes; ensure timely documentation.
