Job summary

Beth Israel Lahey Health is seeking a Physical Therapist to lead home care services, assessing patients, planning care, and coordinating with physicians to deliver high-quality care in patients’ homes and communities.

The role requires a PT degree, NH license, and BLS certification, with 2+ years in PT, ideally in public health or home care. OASIS experience is a plus, and the team values collaboration and patient-centered planning.

Qualifications

• Degree from an accredited PT program; DPT preferred.
• Current NH Physical Therapist license.
• Active BLS certification.
• 2+ years PT experience, with focus in public health or home care.
• OASIS experience preferred.

Responsibilities

• Coordinate care for assigned patient caseload, delegating to PTA staff and supervising if applicable, and complete OASIS time points.
• Complete initial patient and family assessment to plan patient-specific goals and care.
• Initiate and adjust plan of care; perform preventive and rehabilitative therapies as prescribed.
• Communicate with physician about patient needs and changes in condition; obtain orders as required.
• Maintain confidentiality per HIPAA and complete timely documentation.

Education

Degree in Physical Therapy; DPT preferred

NH PT license

BLS certification
