Job summary

Honor Health Network is seeking a dedicated Per Diem Physical Therapist to join our homecare team in Connecticut. You will provide personalized in-home physical therapy services to patients, with a flexible caseload of 6 to 15 visits per week.

Responsibilities

• Deliver high-quality physical therapy services in patients' homes.
• Assess conditions, develop and adjust treatment plans.
• Educate patients and families on exercises and prevention.
• Document assessments, sessions, progress notes, and discharge summaries.
• Coordinate with healthcare providers, caregivers, and family.
• Manage a flexible schedule of 6 to 15 visits per week.
